Q: Does this fit securely in one of the tubes on the side of the Leatherman wave nylon sheath?

No. The "tubes" on the side of my Leatherman Wave nylon sheath are elastic pockets with no bottom. The elastic is not tight around the bit-driver, and it has a small enough diameter to fall through. My solution is to put a pocket clip from an old ink pen onto the bit driver. That eliminates any chance the bit driver can fall through.

Q: Does this convert to regular hex bit size?

Yes you can use regular hex bits in this extension.it has a notch in it to accept the leather man bits that are flat. Those bits are a must have for the leatherman as well. I use them and the extension constantly.
